No requesting records, no waiting for a CD, no scanning. Sign in with the same account you use for your patient portal and your results, conditions, medications, allergies and vaccines come across on their own.
You sign in at your provider
The login happens on their site. open-chart never sees your username or password, and can only read — it can never write anything into your medical record.
Encrypted before we store it
Test names, values, drug names, doses — and the original data your provider sends — are sealed to a key only your password opens. We can’t read the content.
Nothing runs in the background
Records come in when you ask for them. There is no job quietly polling your chart, and disconnecting destroys the credential rather than promising not to use it.

Every Epic health system open-chart is enabled at. Many smaller practices connect through a larger network — if you can’t find your clinic by name, try the hospital system it belongs to.
